Reward Rules
- You can only get your reward points if you have your reward card
- You’ll get your rewards at your regularly scheduled appointments
- Points are not redeemable for cash
- You can purchase a replacement card for 10 points
- Points and prizes are subject to change at anytime
- You’ll get your dental checkup reward points when you show a completed dental certificate
- Points are non-transferable
